CenterWell Senior Primary Care - - Responsibilities: Provide direct patient care to patient under the direction of the RN and according to the Aide Plan of Care; Take accurate temperature, pulse and blood pressure measurements and report abnormal results to supervisor; Assist with personal hygiene including bathing, grooming, skin care, shaving, oral care, nail/foot care; Assist in feeding patients and monitor nutrition and fluid intake; Document interactions and care rendered to patient and communicate with caregivers, doctors and staff
